High Performance Team Analyser

A systemic diagnostic of how your team operates as a living system within a wider ecosystem of stakeholders, commercial pressures and strategic objectives.

This survey draws on Peter Hawkins' Five Disciplines of high-performing teams, integrated with insights from Jim Collins, Stephen Covey, Sir John Whitmore, Amy Edmondson and the wider literature on systems thinking, psychological safety and learning organisations.

You will be asked to rate your team across five domains, alongside a small number of short reflections. The survey takes 15 to 20 minutes. Responses are anonymous by default.

What you will be asked

Rate each statement on a six-point scale from strongly disagree to strongly agree. There are no right answers; honesty is the diagnostic value.

Step 1 of 7

About you and the team

All fields are optional. The information helps with analysis if you submit results. Leave anything blank you would prefer not to share.

If multiple team members complete this survey, use the same team name to enable aggregated reporting.
Optional – used if you choose to receive a copy of your results.
Step 7 of 7

Risk indicators

Patterns in the responses that warrant attention. Each flag is grounded in specific items rather than overall averages, so a single pattern can show up even when domain scores look healthy.

Strengths to build on

Development priorities

Prioritised interventions, organised by where the work needs to happen. Lower-rated items map to the level most likely to move them.

Item-level heatmap

All 50 items, colour-coded by current score. Useful for working sessions; pick three to five items the team agrees most need attention.

Reflections

This report is descriptive, not prescriptive. It is most useful as the starting point for a structured team conversation, ideally facilitated, in which the team makes sense of the patterns together.

Submit your results

Submit your results to your Corryvreckan Consulting facilitator. Provide your email address below if you would like a copy sent to you.
